Diagnostic value of renal ultrasound hemodynamic monitoring for right heart failure

Abstract:

Objective

To investigate the correlation of renal artery resistance index (RRI) with right heart ultrasound parameters, and to evaluate its value in predicting right heart failure.

Methods

Thirty patients with right heart failure and 20 patients with normal right heart function were collected from Zhejiang Hospital during January 2020 and January 2022. The RRI, fractional area change (FAC), tricuspid annular plane systolic excursion (TAPSE), and tricuspid annular plane systolic movement velocity (S′) were measured by ultrasound comparing by t test. The Pearson method was used to analyze the correlations between RRI and FAC, TAPSE, S′; and the diagnostic value of RRI for right heart failure was evaluated through the receiver operating characteristic (ROC) curve.

Results

The RRI level of patients in the right heart failure group was higher than that of the control group (t=9.459, P < 0.01), and the FAC, TAPSE, and S' were lower than those of the control group (r=14.811, 13.269 and 10.912, P < 0.01). Pearson correlation analysis showed that RRI was negatively correlated with FAC (r=-0.8716, P < 0.05) and TAPSE (r=-0.4313, P < 0.05), but has no linear correlation with S' (r=-0.2785, P > 0.05). The ROC curve analysis showed that the area under the curve for RRI to diagnose right heart failure was 0.8250 (95% confidence interval: 0.7121-0.9379, P < 0.01), with the cutoff value of 0.7550, and the sensitivity and specificity were 90% and 60%, respectively.

Conclusion

RRI may be used as a sensitive index for diagnosting right heart failure.
