目的

探讨失眠改善对老年高血压合并失眠患者血压水平的影响。

方法

回顾性分析2018年3月至2020年3月浙江医院收治的老年高血压合并失眠症患者179例。收集患者的一般临床资料、睡眠及血压指标,使用匹兹堡睡眠质量指数量表(PSQI)评估患者睡眠情况。治疗4周时,根据PSQI评分的改善情况将患者分为睡眠改善组和睡眠未改善组,观察失眠改善对患者高血压水平的影响。计量资料的比较采用非参数检验以及t检验,计数资料的比较采用χ2检验。

结果

基线时两组患者收缩压和PSQI总分的差异均无统计学意义(t=0.600,Z=1.531,P>0.05)。治疗4周后,睡眠改善组患者睡眠质量、入睡时间、睡眠时间、睡眠效率以及PSQI总分均显著优于未改善组(Z=4.300、3.613、3.491、5.003、7.829,P<0.05),收缩压差值及舒张压差值均显著高于睡眠未改善组(t=2.274,Z=2.078;P<0.05)。

结论

失眠改善能有效降低老年高血压合并失眠症患者的血压水平,尤其是睡眠时间、入睡时间、睡眠质量和睡眠效率的改善对血压水平影响更大。

Objective

To investigate the effect of improving insomnia on blood pressure levels in elderly patients with hypertension comorbid insomnia.

Methods

Data of 179 hospitalized patients with geriatric hypertension combined with insomnia attending Zhejiang Hospital from March 2018 to March 2020 were retrospectively analyzed. General clinical data, sleep indexes and blood pressure of the patients were collected, and the patients' sleep was assessed using the Pittsburgh Sleep Quality Index (PSQI) scale. At 4 weeks of treatment, the patients were divided into the sleep improvement group and the sleep non-improvement group according to the improvement of PSQI scores, and the effect of the improvement of insomnia on the patients' hypertension level was observed. Nonparametric test and t test were used for comparison of measurement data, χ2 test was used for comparison of counting data.

Results

The difference in systolic blood pressure between the two groups at baseline was not statistically significant (t=0.600, P > 0.05), nor was the difference in total PSQI score (Z=1.531, P > 0.05). The sleep improvement group was significantly better than the non-improvement group in sleep quality, time to fall asleep, sleep duration, sleep efficiency, and total PSQI score after 4 weeks (Z=4.300, 3.613, 3.491, 5.003, 7.829; P < 0.05). And systolic blood pressure difference, as well as diastolic blood pressure difference before and after treatment of insomnia, were significantly higher in the sleep improvement group than in the sleep non-improvement group after 4 weeks (t=2.274, Z=2.078; P < 0.05).

Conclusion

Improvement in insomnia was effective in reducing blood pressure levels in elderly patients with hypertension combined with insomnia, especially the improvement in sleep duration, time to fall asleep, sleep quality and sleep efficiency had a large impact on blood pressure levels.
