老年腰椎管狭窄症患者围术期应用人促红细胞生成素联合蔗糖铁的疗效分析

目的

探讨老年腰椎管狭窄症（LSS）患者围术期应用重组人促红细胞生成素（EPO）联合蔗糖铁的疗效。

方法

选取2019年11月至2021年4月于宁波市第六医院行腰椎后路减压植骨融合内固定术治疗的老年多节段LSS患者100例，其中50例患者围术期予以常规处理（对照组），其余50例患者围术期在常规处理的同时给予EPO联合蔗糖铁治疗（观察组）。比较两组患者的手术情况，以及术前和术后3 d、7 d、1个月的腰部疼痛的视觉模拟评分（VAS）、Oswestry功能障碍指数（ODI）、日本骨科协会评估治疗分数（JOA）。计量资料的比较采用t检验和重复测量方差分析，计数资料的比较采用χ²检验。

结果

与对照组比较，观察组术后Hb最小值和红细胞压积最小值增大、输血量减小、术后下地时间提前、住院时间缩短、输血率降低（t=9.276、8.564、13.673、6.387、9.563，χ²=5.398；P<0.05）。术后两组患者VAS、JOA、ODI均明显改善，但差异无组间效应（F=0.621、1.113、0.564，P>0.05）。

结论

对于老年LSS患者，围术期应用EPO联合蔗糖铁能有效改善贫血状况、降低输血率、缩短住院时间、促进术后早期康复。

Objective

To investigate the effect of perioperative erythropoietin (EPO) combined with sucrose iron in elderly patients with lumbar spinal stenosis.

Methods

One hundred elderly patients with multi-level lumbar spinal stenosis treated by posterior lumbar surgery admitted to Ningbo No. 6 Hospital from November 2019 to April 2021 were selected. Among them, 50 patients were treated with routine treatment during perioperative period (control group), and the other 50 patients were treated with recombinant human EPO combined with sucrose iron during perioperative period (observation group). The surgical situation, visual analogue score (VAS), Oswestry disability index (ODI) and Japanese orthopaedic association (JOA) of lumbar pain before and 3 days, 1 week and 1 month after operation were compared between the two groups. T test and repeated measurement analysis of variance were used for comparison of measurement data, and χ² test was used for comparison of counting data.

Results

Compared with the control group, the observation group showed an increase in postoperative minimum Hb and minimum hematocrit, a decrease in blood transfusion volume, an earlier postoperative time to the ground, a shorter hospital stay, and a decrease in blood transfusion rate (t=9.276, 8.564, 13.673, 6.387, 9.563, χ²=5.398; P < 0.05). The VAS, JOA, and ODI of the two groups improved significantly after surgery, but there was no intergroup effect (F=0.621, 1.113, 0.564; P > 0.05).

Conclusion

For elderly patients with LSS, perioperative EPO combined with iron sucrose can effectively improve anemia, reduce blood transfusion rate, shorten hospital stay and promote early rehabilitation.
