高压氧联合中医定向透药治疗老年糖尿病周围神经病变疗效分析

目的

探讨高压氧联合中医定向透药治疗老年糖尿病周围神经病变（DPN）的临床效果。

方法

选择2020年5—10月于上海市江湾医院收治的老年DPN患者74例，随机分为常规治疗组（40例）和综合康复治疗组（34例）。其中常规治疗组采用常规方法治疗，综合康复治疗组在常规治疗基础上再给予高压氧和中医定向透药治疗。观察治疗前后患者的血糖、血脂等指标及临床疗效。计量资料的组间比较采用t检验，计数资料的组间比较采用χ²检验。

结果

两组患者治疗前后空腹血糖、糖化血红蛋白、总胆固醇及三酰甘油的差异均无统计学意义（P>0.05）。治疗4周后，两组患者麻木、刺痛、灼烧感及刀割样疼痛等临床症状均较治疗前好转，综合康复治疗组密歇根神经病变筛查量表评分明显低于常规治疗组（t=2.451，P<0.05），总有效率明显高于常规治疗组（χ²=4.010，P<0.05）。

结论

高压氧联合中医定向透药疗法可以明显改善DPN患者的机体神经功能，提高临床疗效。

Objective

To investigate the clinical efficacy of hyperbaric oxygen therapy combined with directional drug penetration treatment of traditional Chinese Medicine on diabetic peripheral neuropathy (DPN) in elderly patients.

Methods

Seventy-four elderly patients with DPN treated in Shanghai Jiangwan Hospital from May to October 2020 were recruited as research subjects, and were randomly divided into conventional therapy group (n=40) and comprehensive rehabilitation group (n=34). Patients in conventional therapy group were treated with conventional therapy, while those in comprehensive rehabilitation group were treated with hyperbaric oxygen and directional drug penetration treatment of traditional Chinese Medicine in addition. The blood glucose, blood lipid and clinical efficacy of the patients were observed before and after treatment for 4 weeks. T test was used for the comparison between measurement data, and chi-square test was used for the comparison between counting data.

Results

There was no statistical difference in fasting blood glucose, glycosylated hemoglobin, total cholesterol and triglyceride between the two groups before and 4 weeks after treatment (P > 0.05). After 4 weeks of treatment,the clinical symptoms such as numbness, tingling, burning sensation and knife-cutting pain in both groups were improved compared with those before treatment, and the Michigan neuropathy screening instrument score of the comprehensive rehabilitation group was significantly lower than that of the conventional therapy group (t=2.451, P < 0.05). The total effective rate of the comprehensive rehabilitation group was significantly higher than that of the conventional therapy group (χ²=4.010, P < 0.05).

Conclusion

Hyperbaric oxygen combined with directional drug penetration treatment of traditional Chinese Medicine can significantly improve the nervous function of elderly patients with DPN and improve the clinical therapeutic effect.
