肾细胞癌(RCC)是泌尿系统常见恶性肿瘤,其早期症状隐匿、异质性强,部分患者术后易复发转移。传统组织活检和影像学检查在动态监测及肿瘤异质性评估方面存在局限性,液体活检具有无创、可重复和可动态检测等优势,已成为精准诊疗的重要研究方向。循环肿瘤细胞、循环肿瘤DNA、外泌体及非编码RNA均为RCC液体活检的重要标志物。其中,环状RNA、长链非编码RNA和微小RNA因稳定性较高、调控功能明确,在RCC诊断、疗效监测及预后评估中显示出较好的应用前景。本文结合国内外最新循证医学证据及临床实践探索,重点围绕非编码RNA在RCC液体活检中的研究现状及临床价值进行梳理和总结,形成5点共识意见,旨在为我国泌尿外科医师防治RCC提供更明确的参考和更规范化的建议,以期进一步提高患者获益。

Renal cell carcinoma (RCC) is a common malignancy of the urinary system, characterized by insidious early manifestations, marked heterogeneity, and a high risk of postoperative recurrence or metastasis in some patients. Current expert consensus indicates that conventional tissue biopsy and imaging modalities have inherent limitations in dynamic disease monitoring and the assessment of tumor heterogeneity. Due to its minimal invasiveness, repeatability, and capacity for real-time dynamic evaluation, liquid biopsy has become an important focus in precision oncology. Circulating tumor cells, circulating tumor DNA, exosomes, and non-coding RNAs are regarded as key liquid biopsy biomarkers in RCC. Among these, circular RNAs, long non-coding RNAs, and microRNAs have attracted increasing attention because of their relative stability and well-characterized regulatory functions, demonstrating considerable potential in the diagnosis, treatment monitoring, and prognostic assessment of RCC. Based on the latest evidencebased evidence and clinical practice from China and abroad, this consensus summarizes the current research and clinical utility of noncoding RNAs in liquid biopsy for RCC, and formulates five key consensus statements. These are intended to offer Chinese urologists more explicit references and standardized advice for the prevention and treatment of RCC, thereby further enhancing patient benefit.
